PARENTAL CONSENT FORM

The following student has my permission to use the limousine indicated below for transportation during the indicated date for transportation to and from the prom including other locations as agreed between the responsible parent and Allpoints Corporate Express.  The following rules will be enforced during the rental period.

  1. The student who's name appears below is responsible for the behavior of all of the guests in the limousine during the rental period.

  2. No alcoholic beverages or illegal substances will be in or around the limousine or consumed by the passengers during the rental period.

  3. The chauffeur has expressed permission to inspect any bags, back packs or other containers brought onboard by the guests for illegal substances.

  4.  The moon roof, if so equipped, may not be stood up in while the vehicle is under way.

  5.  The parent listed below must be available on the phone number supplied during the rental period.

  6. The parent listed below will be responsible for any damages to the limousine beyond the usual use of the limousine.

  7. The parent listed below will be responsible for the transportation of all limousine passengers if the contract is voided due to non compliance with this agreement.

  8.  Termination of this agreement will occur if in the opinion of the chauffeur there is a serious safety problem or the conditions of this agreement are violated.
