The student whose name appears below will be responsible for the
behavior of all guests in the limousine during the contracted
period.
No alcoholic beverages or other prohibited substances will be
brought into the vehicle.
The driver has the right to inspect any bags, containers, or
backpacks for prohibited substances.
The moon roof may not be used when the vehicle is under way.

A contact parent’s name and phone number MUST be provided to the
driver. Contact parent needs to be available by phone for the length
of time of the contract. In the event of a breach of the conditions
stated in this form the contact parent will be responsible for
providing transportation for students.
Termination of this contract will occur in the event of ANY
breach of this agreement and all moneys shall be forfeited and the
trip canceled immediately.
All prom packages
are for a minimum of six consecutive hours.
